To be considered for a license to provide medical or non-medical home health service as a home health agency or nonagency provider, you must submit a completed application and a non-refundable application fee of $250.00. To submit both the application and the electronic payment, you will need to choose both the Application (PDF) and online payment link separately.  After selecting the payment link, you will need to select “No” for the question “Are you paying for an Invoice?”.

The Ohio Department of Health (ODH), for purposes of  Non-medical Home Health Services licensure, requires the actual fingerprint card or a printed copy of the fingerprints. ODH is not asking for the results of a background check. Most livescan locations do not have the ability to print an individual’s fingerprints so if you choose livescan, please verify you can receive a copy of your prints at your location. Fingerprints may be taken by most local law enforcement agencies, including your local police department or sheriff’s office. You are encouraged to contact your local enforcement agency to make arrangements for obtaining your fingerprints. You may be charged a fee by the agency for the “rolling” of your fingerprints. You must have the law enforcement officer rolling your fingerprints sign and date the fingerprint cards in the designated space.

The requirements of Chapter 3740. of the Ohio Revised Code (ORC) included in House Bill 110 of the 134th General Assembly, pertaining to the licensing of Home Health Agencies and Nonagency Providers are currently under development. Section 3740.02 ORC requires implementation of licensure beginning one year after the effective date of the legislation, which was September 30, 2021.
